What is Support Coordination?

At times it can be difficult finding and coordinating the support that best suits your needs. Our experienced team will work with you to navigate the NDIS, identify your goals and connect with services that will help you to reach those goals.

Together, with you, your family and your community we assist you to better understand and manage your NDIS plan so you can meet your needs and achieve your goals.

What are the benefits of Support Coordination?

Support coordination can help you build lifelong networks, skills, and connections to help you become – and remain – independent. It is particularly useful during periods of transition in your life – such as starting school, university or work or moving to a new home or area.

What do Support Coordinators do?

Broadly speaking, Support Coordinators support you to work with your family and your community to:

  • Understand your funding
  • Set up your initial bookings and service agreements with your providers
  • Access the community supports that you need and want
  • Connect with appropriate supports and community networks
  • Develop practical skills such as budgeting and life planning
  • Support you to avoid and resolve crises.

At Thrive Access and Support, your dedicated NDIS Support Coordinator will work to:

 

Understand your needs

Your Support Coordinator will work with you to get to know you and understand what is important to you so they can best identify your goals and what you need support with.

Help you to implement your NDIS plan

Your Support Coordinator will support you to better understand and implement your NDIS plan. This includes:

  • helping you find, choose and access service providers that best fit your needs.
  • working with you to set up your service agreements and your initial bookings with them.

Assist you to budget

Your Support Coordinator will work with you to:

  • develop your confidence, skills and knowledge to manage and monitor your own NDIS plan budget.
  • teach you to access and use the NDIS portal, so you can use it to access your NDIS information.
  • build your understanding of the NDIS price guide, so you can learn about the cost of NDIS supports.

Link you to mainstream supports

Our Support Coordinator will help you to find and use appropriate mainstream supports in your community. These include non-NDIS funded supports such as:

  • homelessness
  • justice
  • health
  • education
  • social activities

Review and maintain your NDIS plan

Your Support Coordinator will support you to:

  • regularly review your NDIS plan.
  • work with your service providers regarding referrals.
  • assist you to identify barriers and resolve problems.
  • Report to the NDIS regarding the implementation of your plan at eight weeks and prior to your plan ending.

Support your independence

We will support you to build your skills, confidence and knowledge so that you can learn as much about the NDIS as you are able to.
The ultimate goal is for you to gain as much independence as possible when it comes to managing your NDIS supports.

Code of conduct

Your Support Coordinator must adhere to the NDIS code of conduct and will report any areas of concern such as:

  • abuse
  • neglect
  • restrictive practices
  • child safety
  • self-harm

What a Support Coordinator cannot do

It is important to know that there are some things a Support Coordinator is unable to do for you. These include:

  • Transporting you to and from your appointments.
  • Signing service agreements on your behalf.
  • Providing Case Management support.
  • Providing direct personal support.
  • Managing your budget.
  • Booking carer shifts, medical shifts etc beyond your initial service appointments.
  • Ordering continence products for you.

Who can access Support Coordination

People living with disability, mental health or who simply need extra help due to age, illness or injury.

What does a Support Coordination session cost?

Standard Support Coordination consultation – $100.14 per hour.
